All accommodations must be approved by the Student Disability Resource Center.
SDRC reserves the right to deny service or accommodations until appropriate documentation is provided.
All documentation must be TYPED on full-size letterhead and signed by a clinician. It must include your full name, date of birth, diagnosis, diagnostic code, and recommended accommodations. Documentation must be provided prior to receiving services.

A statement from a diagnosing physician or psychologist stating the disability (DSM-V or ICD-10 codes) and functional limitations
Your most current Psycho-educational evaluation, not your IEP
Audiologist’s report, including the diagnosis, the nature of the disability, severity, and functional limitations
Ophthalmologist’s statement, including the diagnosis, the nature of the disability, severity and functional limitations
A statement from a Mental Health Professional (psychologist or psychiatrist) that includes diagnosis (DSM-V or ICD-10 codes), nature of disability, severity and functional limitations

Copy of the Accommodation Form from home Institution will be considered.
If you received Special Education services while in the public school system (Itinerate, Resource or Self-Contained), we will need your most current Psycho-Educational Evaluation (rather than an IEP) or your 504 Plan.
The Psycho-Educational Evaluation is the evaluation that determined that you were eligible for services in Special Education. This is not your IEP. Your IEP will tell us WHAT you received in public school, it will not tell us WHY. Your evaluation will assist us in providing the most effective accommodations and services.

When it is not obvious what service an animal provides, only limited inquiries are allowed. Employees may ask two questions:
Employees cannot ask about the person’s disability, require medical documentation, a special identification card or training documentation for the service animal, or ask that the service animal demonstrate its ability to perform the work or task.
A common disability is an allergic reaction to animals. Persons making an asthmatic/allergy/medical complaint are directed to the Student Disability Resource Center and must show medical documentation to support the problem as efficiently and expeditiously as possible.
In the event the situation cannot be resolved, the Dean of Students office will assist within the grievance procedure.
The Americans With Disabilities Act and its amendments define a service animal as a dog that is individually trained to do work or perform tasks for an individual with a disability. Under Title II and III of the ADA, dogs are recognized as service animals.
Therapy and Companion Animals are generally not covered under the ADA.
